CHOICE Care Coordination

Certified by Medicaid, a care coordinator is an independent provider who screens and prepares written assessments for persons accepted to the Medicaid CHOICE Waiver program. Following the assessment and completion of the plan of care, the care coordinator continues to provide phone and face-to-face contacts with the client. If changes in services are needed, the care coordinator is responsible for amending the plan of care. Senior Citizens of Kodiak, Inc. has certified care coordinators to help with home and community based services for those in need and who qualify for Medicaid CHOICE Waiver and also for those who do not qualify.
